On the corner of Cheyenne and Tenaya Way between MountainView Hospital and the parking garage on Cheyenne. Suite 110 is in the one story portion of the building, not the main building.

On the east side of Pecos Road between Sunset Road and Post Road in the Gateway Business Park. The office is located in the second building to the east, behind the building facing Pecos Road.

North of Smoke Ranch Road between Buffalo Drive and Tenaya Way. Take the second right. The office is in the last suite on the right. The Veteran's Affairs building is diagonally across the street.

On Rainbow Blvd. and West Sunset Road in the same parking lot as the Rockin Baha Lobster restaurant.
